Confirm training course details

Thank you for registering for a senior mental health lead training grant. To receive the grant, you must provide us with information about the Department for Education (DfE) quality assured training course you've booked.

If you're not the senior mental health lead, you'll need to work closely with them to complete this form.

Before you start, you'll need:

• the course code
• evidence of the booked DfE quality assured course - this could be a scanned copy or photograph of your confirmation email or invoice from your training provider
• details of how you identified the course
• the cost of the course
• up-to-date bank account details (opens in a new tab) to receive your payment (opens in a new tab).

You must complete this form in a single session. If you're inactive for more than 15 minutes, you'll need to start the application again.
